How To Make a Referral

Montana Chemical Dependency Center
is the single state chemical dependency treatment facility administered by the Department of Public Health & Human Services, Addictive & Mental Disorders Division. It is a 76 bed in-patient treatment facility operating 24 hours per day, 365 days per year.

  • Only Montana State Licensed Addiction Counselors may submit referrals to MCDC
  • A referral packet must include a current biopsychosocial assessment, a Level of Care Index which meets the American Society of Addictive Medicine (ASAM) Level III.7 Patient Placement Criteria and releases of information from the referral sources, medical, mental health and legal professionals.
  • Referral packets are submitted to MCDC Admission's Supervisor.
  • Complete referral packets, which meet appropriate placement criteria, will be accepted for treatment at MCDC and scheduled for admission.

Generally, all admissions are assigned arrival times between Monday through Friday 8:00 a.m. to 8:00 p.m. Unique circumstances warranting alternative admission times may be requested and will be accommodated as much as possible.
